WAIVER AND RELEASE OF LIABILITY 
 
In consideration of being allowed to participate in any way in one of our MAKS LEAGUES, related events and activities, the registrant acknowledges, appreciates and agrees that:
  • The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment and personal discipline may reduce this risk, the risk of serious injury does exist; and,
  • I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for this participant; and,
  • I willingly agree to comply with the stated and customary terms and conditions for participation.  If however I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately;
  • I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS, 3on3 Hockey Hamilton, their officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.
  • I consent to the use of my child’s name (as entered above) to appear on the MAKS LEAGUES website for the purposes of listing on roster and stats on each individual league on our MAKS LEAGUES website.

This is to certify that I, as parent/guardian with legal responsibility for this participant, do consent and agree to his/her release as provided above of all the Releasees, and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify the Releasees from any and all liabilities incident to my minor child's involvement or participation in these programs as provided above.

Privacy Policy

Pro Hockey Life Academy's policy is to respect and protect the privacy of our users. This policy statement tells you how we collect information from you and how we use it. Using the Internet should be a fun experience and we want to help you make it so. Except in the limited circumstances stated below, phlacademy.ca and 3on3hockeyhamilton.ca will never intentionally disclose individually identifying information about its members to any third party without that member's permission.

Where do we gather information from you and what do we do with it?
When you sign up through our Ecommerce and User Interface System, we require that you enter your full name and e-mail address. We may also ask you for additional information like your address, or other pieces of information to match MAKS Leagues records. Additional information may be collected and/or required for the administration of some of our programs and services.

We offer online registration for programs and services offered by MAKS Leagues.  The information gathered through the registration process is stored and only relayed to those who administer these programs. Some of these registration processes include e-commerce transactions. These are done through an external gateway ("Internet Secure") on behalf of PHL Leagues. In order to verify the identity and provide ample security for our users, address and/or postal code information is supplied to this company to compare and authorize financial settlements. PHLA Leagues does NOT record any credit card numbers through this process, only the confirmations of approval - which are added onto receipts for the product/service.

PHLA Leagues issues and reads cookies to track any customization options you may have selected. A cookie is a small file that is stored on your computer which holds information that you have asked our website to keep track of. Most browsers are initially set up to accept cookies. You can reset your browser to refuse all cookies or to indicate when a cookie is being sent.  

We also keep track of how phlcademy.ca is accessed. When you request a page from our servers, we store that request and the time you made it, along with the IP address of the computer that made the request. (An IP address is a unique number assigned to every computer on the Internet that other computers, like our servers, use to identify and send data to that computer.) We keep track of these requests in pretty much the same way that every commercial Web site out there does. This access information helps us figure out what the most and least popular parts of the site are, so we can make more of the most popular things and less of the least popular things.

How else do we use the information that is gathered?
We present reports, to potential and current advertisers about what kinds of people will see or have seen their ads. Remember that without your consent, we never tell advertisers information that individually identifies you, such as your name or your e-mail address.

We may, however, disclose information that individually identifies you to third parties when we have reason to believe that disclosing this information is necessary to identify, contact or bring legal action against someone who may be causing injury to or interference with (either intentionally or unintentionally) our rights or property, other members or users of laurierathletics.com, or anyone else that could be harmed by such activities. We may also disclose such information to third parties when we believe in good faith that the law requires it.

Refund Policy

If you are cancelling your registration prior to the season starting you will receive a full refund less the $25 administration fee. Once the teams have been organized, and the schedules are posted online within two weeks of the season starting you will only receive a 50% refund. There are no refunds once the season has started. Any changes to a registration a $25 administrative fee will apply.
